The mission of the Smiths Falls Nurse Practitioner-Led Clinic is that of ensuring health promotion, illness prevention and chronic disease management for the people of Smiths Falls and surrounding counties of Leeds, Grenville and Lanark, through improved access to exemplary comprehensive patient-focused primary health care services.

We acknowledge that since we are located in the town of Smiths Falls, we are gathering on the traditional, unceded territory of the Algonquin Anishanaabe people, whose ancestors have lived here since time beyond memory, before colonization, and still today have rights in these lands and waters.
With gratitude we recognize that the culture, presence, and stewardship of the Algonquin Peoples have nurtured the lands and the waters of this region for many generations, continue to do so today, and will for many generations to come.
We extend our friendship to all Indigenous, Inuit, and Metis Peoples, and commit to reconcile to one another by caring for each other, and for future generations with mindfulness.
