The clinic is closed over the lunch hour from 12-1pm.

Phone Hours: Monday to Thursday from 9am until 11am and 1pm to 3:30pm and Friday from 9am to 11:00am.

Updated Clinic Hours: Monday to Thursday from 9am until 4pm and Friday 9am until 12noon.

If you have paperwork to drop off you can utilize our secure drop box on the front of our building. Ensure your information has been completed including any spots you are required to sign or we will request you return to complete these.

Change on Masks
If you are feeling unwell and/or have symptoms such as fever, sneezing or coughing, we ask that you continue to wear a mask for your appointment; even if you think, “It’s just allergies” …  The Smiths Falls Nurse Practitioner-Led Clinic is no longer requiring clients who feel well to wear a mask for their appointment. If you feel more comfortable continuing to wear a mask, feel free to bring one to wear during your appointment.

If you are running out of medication, please ask the pharmacy to fax the renewal request to the Smiths Falls Nurse Practitioner-Led Clinic. Our policy is that you allow 7 days for renewal requests to be completed.

 

Online Booking!

Clients registered to the Smiths Falls NP-Led Clinic can now book appointments online (valid OHIP card number needed). Please note that you may book in to see your Nurse Practitioner. 

The online booking is monitored by administrative staff, and you may receive a call to confirm the details.

IMPORTANT: If you currently have an appointment scheduled, please call reception they can assist you with changing the date and/or time.

Nurse Practitioner-Led Clinics are a collaborative team based health care model in which Nurse Practitioners are the lead providers of primary health care. These clinics were created to provide accessible, quality family health care services to people that are unattached in their community.

What is a Nurse Practitioner?

A Nurse Practitioner is a Registered Nurse with advanced training and skills in the areas of health assessment, diagnosis, and treatment of common episodic illnesses as well as chronic disease.  The Nurse Practitioner works within a scope of practice set forth by the College of Nurses of Ontario. Their practice is based on a holistic approach and they work collaboratively with many other health professionals.

What Services Can Nurse Practitioners Provide?

  • Diagnose and treat illness and/or injuries
  • Perform physical check-ups
  • Pap tests
  • Immunizations
  • Order and interpret diagnostic tests
  • Write prescriptions and monitor chronic conditions
  • Provide treatments and/or procedures
  • Make referrals to specialists
  • Order MRI and CT scans

 

 

 

 

A Team-Based Approach

Our Clinic operates with a team based approach. In addition to four nurse practitioners, our health care team includes:

Registered Nurse: The Nurses will be using their full scope of practice. They will conduct health care assessments including meet and greet visits, focused assessments, pap tests, immunizations, lab tests, well baby, well mom, and health promotion/education.

Registered Dietitian: The dietitian uses food and nutrition knowledge to provide practical advice to help promote health and wellness and manage chronic diseases.

Social Worker: The social worker provides counseling support to patients in our clinic addressing such issues as; depression, stress/anxiety, chronic illness/pain, grief/loss and poverty.

Pharmacist: The pharmacist provides medication reviews, identifies medication interactions and provides general education.
